Initial Triage & Intake:
Before your appointment date, our intake officer will contact you to register your details and gain an overview of your diagnosis and situation so that our Dementia Support Specialist is fully prepared to best serve you during your time together.

Caring Conversation and Post-Diagnostic Support
During your first appointment with our Dementia Support Specialist, you’ll engage in a warm and supportive conversation where your story and concerns can be expressed and heard, and any questions answered. Whatever it is that matters most to you and identifying the greatest needs for you and your loved one.

This includes things like unpacking and giving you valuable information about the specific type of dementia or cognitive change that you or your loved one have been diagnosed with, along with practical support strategies, and help with future planning.
Once a clear understanding of your situation, has been established we will provide a comprehensive support plan tailored to you and your family.

We will also gift you with a welcome pack, and helpful information resources, tailored especially to your unique situation. We’re here to guide you every step of the way.
